Qualifications

• AHA BLS - Healthcare Provider certification is required.
• Minimum of an Associate's degree in nursing; BSN preferred.

Responsibilities

• Triage and assess participants in home visits and provide education resources.
• Participate in the interdisciplinary team to develop plans of care.
• Conduct comprehensive nursing assessments and document care provided.
• Communicate changes in health status to the IDT and coordinate 24-hour care delivery.
• Support quality initiatives and participate in QAPI activities.
