Job summary

Capital Health in New Jersey is seeking an experienced Registered Nurse to provide in-home nursing care and virtual monitoring for the Hospital at Home program. The role emphasizes patient safety, education, and coordinated care with the on-site command center.

The position requires three years of acute care experience, NJ RN or multi-state license, and certifications in ACLS and BLS, with NIH Stroke Scale certification or the ability to obtain it within orientation.

Qualifications

• Associate degree or nursing degree required; BSN preferred.
• Three years of acute care experience; telemetry experience required.
• RN license in NJ or multi-state; ACLS and BLS certifications.
• NIH Stroke Scale certification or eligibility by orientation; annual stroke-related CNE.

Responsibilities

• Deliver nursing care in-home to patients enrolled in the Hospital at Home program.
• Conduct virtual nursing assessments via video conferencing from the Command Center.
• Monitor patient status remotely using telemetry devices and EMR data.
• Triages patient needs and escalates as appropriate to ensure safety and quality.
• Participate in discharge planning and patient education for medications.
