Job summary

Trinity Services in Naperville, IL seeks a Social Rehab Counselor to provide clinical and residential support under supervision. You will assist individuals with daily living, transportation, and coordination with treatment teams, while maintaining Medicaid billing and documentation.

This role requires a Bachelor’s degree and the ability to work overnight Sunday–Thursday. The position reports to the Team Leader and is part of Network 4.

Qualifications

• Bachelor’s Degree in Social Work, Psychology, or related field.

Responsibilities

• Provide community support, including transportation to day programs, medical, benefits, or employment appointments.
• Deliver case management services and coordinate with therapists and treatment teams.
• Support individuals with medication management and daily living skills.
• Maintain accurate service documentation and Medicaid billing.
• Attend monthly supervision meetings and collaborate with treatment teams.
