Job summary

Southwest Behavioral Health Services in Arizona is seeking a Direct Care Provider for our residential programs. You will perform screening, assessment, treatment planning, life skills coaching, pre-vocational training, related documentation, case management, crisis intervention, and individual or group counseling for seriously mentally ill clients in a residential setting.

Schedule is Wednesday-Saturday, 9pm-7am with shift differential.

Qualifications

• Minimum age 22.
• High School Diploma or GED with six months full-time behavioral health experience.
• Associate’s degree with six weeks full-time behavioral health experience or related qualifications.

Responsibilities

• Provide screening, assessment, and treatment planning for residents.
• Deliver life skills coaching and pre-vocational training.
• Conduct crisis intervention and individual/group counseling; document care and progress.
• Coordinate case management and related documentation in residential settings.
