Job summary

Southwest Behavioral & Health Services in Arizona is seeking a Direct Care Provider to join our residential programs. You will deliver screening, assessment, treatment planning, life skills coaching, and crisis intervention for seriously mentally ill consumers in a residential setting.

The role requires a high school diploma or GED and relevant experience, with evening shift coverage. Salary is determined by shift and experience; we offer PTO, holidays, comprehensive benefits, retirement

Qualifications

• Minimum age 22 years.
• High School Diploma/GED with 6 months BH experience, or an Associate’s degree with 6 weeks BH experience, or a bachelor’s degree with relevant experience.
• MVR and fingerprint clearance as required.

Responsibilities

• Screening, assessment, and treatment planning for seriously mentally ill consumers in a residential setting.
• Deliver life skills coaching, pre-vocational training, and related documentation.
• Provide case management and crisis intervention, individual and/or group counseling.
