Community Health Resources, Inc. (CHR) Willimantic program seeks a Residential Aide to join Heartwood Home in Willimantic, CT. This full-time role supports a 12-bed recovery residence, operating 24 hours a day.

You will assist residents with daily living skills, medication monitoring, group activities, and coordination with staff to maintain safety and program standards. CPR/First Aid certification or training within 90 days is required.

Qualifikationen

• Education: High School Diploma or equivalent.
• Experience: Experience in Human Services with responsible work history.
• Licensure/Certification: Valid driver’s license with good driving record; CPR & First Aid Certification required OR must obtain within 90 days of employment.

Aufgaben

• Maintains physical environment and program standards to CHR, DMHAS, JCAHO and licensing standards.
• Monitors medications of all residents in accordance with policy.
• Teaches and assists residents in rehab areas: nutrition, healthy living, meal preparation, hygiene, laundry, budgeting, and basic home care skills.
• Assists/Manages administrative functions and tasks as directed by supervisor.
• Prepares and facilitates educational and skill-building groups and one-to-one interventions as directed by the treatment plan.
• Collaborates and coordinates care with providers and families as needed.
• Assists residents in addressing medical and psychiatric needs.
• Maintains documentation in compliance with agency and program standards.
• Participates in staff meetings, supervision, trainings and other meetings as assigned.

Kenntnisse

CPR & First Aid Certification

Valid driver’s license

Human Services experience
