Responsibilities

• Provide direct resident care per the plan of care under supervision of Charge Nurse.
• Assist with feeding, mobility, hygiene, and daily living activities per protocols.
• Monitor intake, hydration, and weight; document and report changes.
• Support continence care and restorative activities as directed.
• Maintain a clean, safe, and respectful environment for residents.
• Communicate effectively with residents, families and staff; participate in shift handoffs.
