Job summary

The Pennant Group, Inc. is seeking an experienced Hospice Registered Nurse (RN) for on-call, overnight and weekend response to support patients in homes and care facilities across Colorado.

The role emphasizes in-person nursing interventions, compassionate end-of-life care, and responsive triage to manage pain, symptoms, and crises. Candidates must hold an active Colorado RN license, have at least two years of nursing experience, and be willing to travel overnight.

Qualifications

• Current Colorado RN license in good standing.
• Minimum 2 years nursing experience.
• Hospice, palliative care, ER, ICU, oncology, or home health experience preferred.
• Valid driver’s license, reliable transportation, and proof of insurance.
• Ability and willingness to travel overnight to patient homes/facilities.

Responsibilities

• Respond to after-hours hospice calls and provide in-person nursing care during overnight hours.
• Perform skilled nursing assessments in homes/FAC during overnight hours.
• Manage pain crises, symptoms, respiratory distress, and other urgent clinical concerns.
• Educate caregivers on medications, comfort measures, and symptom management.
• Complete admissions and after-hours hospice documentation and care planning.
