Job summary

Citizens Options Unlimited in Plainview, NY is seeking a Direct Support Professional to support residents in daily living, health, safety and overall wellbeing. You will promote the agency’s mission in every interaction and collaborate with the team to provide high-quality support.

The role includes overnight and morning care, assistance with meals and routines, safe driving for transportation to programs, and adherence to AMAP policies and documentation.

Qualifications

• High school diploma or GED preferred.
• New York State driver’s license and agency insurance considerations.
• Successful completion and maintenance of AMAP status.
• Ability to communicate clearly and work well in a team.

Responsibilities

• Support people in daily routines, health, safety and wellbeing.
• Assist with overnight and morning care (self-care tasks, meals, toileting, showering, dressing).
• Help maintain a safe, clean home and participate in household chores.
• Follow treatment plans and use adaptive/safety equipment as needed.
• Document events; report abuse/neglect and incidents per policy.
• Maintain compliance with regulations and AMAP procedures.
• Assist with transportation to day programs or job locations.
• Interact with the interdisciplinary team and mentor new staff.
• Perform other duties as requested by supervisor.
