Citizens Options Unlimited is seeking a Direct Support Professional to provide overnight and morning care, ensuring health, safety, and a high quality of life for individuals supported. The role includes personal care, meal support, medication assistance per AMAP policy, and documenting daily activities.

Qualifications

• Ability to communicate and understand written and oral directives/information.
• Excellent team player with a positive attitude.
• Qualified New York State Driver’s license.
• Successful completion and maintenance of AMAP status.

Responsibilities

• Provide overnight and morning support with self-care routines (toileting, feeding, showering, dressing and diapering).
• Maintain health, safety and well-being of people served; promote quality of life.
• Assist with household chores and safety, and participate in chores as needed.
• Follow treatment plans and assist with medication administration per AMAP policy.
• Report incidents and events that affect health or safety to supervisors; document activities.
• Accompany or transport people to day programs or job locations using safe driving practices.
• Mentor new Direct Support Staff and participate in team meetings.
