Job summary

Beth Israel Lahey Health is seeking an Overnight Hospice LPN to provide professional nursing care within the hospice team under supervision of the Hospice RN Case Manager. You will assess, plan, implement, and evaluate nursing services as part of the established plan of care for assigned patients.

The role emphasizes compassionate care, symptom management, and collaboration with physicians, nurses, and therapists to achieve hospice outcomes while honoring patient and family goals and values.

Qualifications

• Active Massachusetts LPN license.
• NLNAC-accredited LPN program graduation.
• Active American Heart Association BLS certification.
• 2+ years of LPN experience in acute care, rehab or home care.
• 1+ year hospice experience preferred.

Responsibilities

• Perform nursing visits to patients as assigned by the Hospice RN Case Manager/Hospice Clinical Manager.
• Provide hospice nursing care to patients and families per palliative care principles.
• Observe, record, and report significant changes to the MD and Hospice RN Case Manager.
• Participate in health teaching for patient and family.
• Respect patient and family beliefs and treatment preferences.
• Communicate effectively with patients, caregivers, and the interdisciplinary team.
• Attend regular IDT meetings.
• Maintain confidentiality per HIPAA and agency policies.
