Job summary

Beth Israel Lahey Health at Home is seeking an LPN to join our hospice team in a full-time day role. You will perform nursing visits, provide hospice care, and communicate with physicians under the supervision of the RN Case Manager.

The successful candidate will hold a Massachusetts LPN license, maintain BLS certification, and have at least two years of LPN experience in acute care, rehab, or home care, with hospice experience preferred.

Qualifications

• Must hold a Massachusetts LPN license and current BLS certification.
• Minimum two years as an LPN in acute care, rehab, or home care; hospice experience preferred.

Responsibilities

• Perform nursing visits to patients as delegated by the Hospice RN Case Manager/Clinical Manager.
• Provide hospice nursing care using palliative care principles to achieve hospice outcomes.
• Observe, record, and report significant changes in patient health to the MD and RN Case Manager.
• Communicate with the physician to report changes and obtain orders as required.
• Participate in weekend/holiday rotation as defined by the organization.
• Document services using a problem-oriented record; participate in hospice case conferences as required.
