Job summary

Beth Israel Lahey Health at Home is seeking an Overnight Hospice LPN in Reading, MA to provide professional nursing care under the supervision of the Hospice RN Case Manager. You will perform nursing visits, deliver hospice care, and collaborate with the team to meet patient goals and comfort.

Requirements include MA LPN license, NLNAC-accredited program, active BLS, and 2+ years in relevant settings; hospice experience preferred. This is a night shift with competitive pay and benefits.

Qualifications

• Active nursing license in MA (LPN).
• BLS certification required.
• 2+ years as LPN in acute, rehab, or home care.

Responsibilities

• Perform nursing visits to assigned patients.
• Provide hospice nursing care following plan of care.
• Observe and report changes to MD/UMC Hospice RN Case Manager.
• Participate in patient and family health teaching.
