New Horizons Resources Inc is seeking a Direct Support Professional for overnight shifts in Amenia, NY. You will support individuals with safety, life-skills training, relationship-building, and community participation according to each person’s Life Plan.

Responsibilities include daily plan implementation, documentation in electronic health records, and assisting with health, hygiene, transportation, and activities.

Formación

• Ability to physically assist individuals as required; and, to intervene physically when appropriate using approved techniques to safeguard the individual, oneself and others during behavioral crises.
• Ability to relate appropriately to individuals with DD, their families and their advocates; and, to constructively relate to and communicate with peers and supervisors and to be able to function successfully as part of a team.
• Successfully complete all agency-required training within six months of employment.

Responsabilidades

• Actively participate in the development of each person’s total plan of support; this may include participation in the person’s Life Plan or CFA meeting; providing feedback and input on Staff Action Plans; completing monthly notes and clinical documentation as required.
• Be responsible for the daily implementation of all goals and supports and the required documentation. Employees in this position are responsible for recording the necessary documentation on an Electronic Health Record, Precision Care or other software applications utilized by NHR.
• Assist and supervise each person as needed in achieving their identified valued outcomes. This may include (but not be limited to), assisting and supervising an individual in participating in activities such as banking, budgeting, shopping, household chores, leisure and social skills training.
