Responsibilities

• Supports individuals with disabilities in daily living tasks.
• Track individuals’ goals, support plans and outcomes; document information as needed.
• Communicate and self-advocate effectively with doctors and other professionals.
• Participate in household chores, routines and community activities.
• Adhere to Individualized Support Plans and state regulations.
