Seven Hills Foundation in Wareham, MA is seeking a Direct Support Professional II- Asleep Overnight to empower individuals to live independently in their communities. You will provide daily care, follow ISP/Behavior Support Plans, assist with medications, and help build life skills during overnight shifts.

This role offers a Monday-Thursday 11:00pm-9:00am schedule with asleep hours paid at $17.50/hour, and location-based differentials.

Responsibilities

• Provide daily support and care to individuals in a residential setting.
• Follow and implement Individual Support Plans (ISP) and Behavior Support Plans.
• Assist with personal care, medication administration, and health-related needs.
• Support individuals in community activities, relationships, and social connections.
• Accompany individuals to appointments and ensure overall health and well-being.
• Maintain accurate documentation and data collection to track progress and outcomes.
• Take a leadership role by training and mentoring other staff.
