Job summary

REM Community Services, a part of Sevita, is hiring a Direct Support Professional in Wisconsin to provide person-centered supports in residential settings. The role supports 4-6 individuals in a program and up to 10 in periodic programs, with a focus on dignity and independence.

You will assist with daily living activities, health maintenance, medication administration, and community integration, while documenting progress and maintaining confidentiality.

Qualifications

• High school diploma or equivalent preferred.
• Six months of experience in human services preferred.
• Must be 18 years of age.
• Working knowledge of computers.

Responsibilities

• Deliver support per individual service/program plans, including socialization, health maintenance, medication administration, skill/behavioral development.
• Assist with daily activities: Personal care, housekeeping, recreational activities, transportation, community integration, shopping, citizenship, and other activities of daily living.
