Job summary

NYC Department of Health and Mental Hygiene is seeking a Community Coordinator to oversee the Relay program across hospital EDs and link patients to community services. You will train ED staff, collaborate with partners, and use your lived experience to support individuals after non-fatal overdoses.

The role requires at least two years in human services, readiness to travel the five boroughs, and a commitment to harm-reduction approaches. Residency within NYC is expected.

Qualifications

  • A bachelor’s degree from an accredited college and two years of experience in community work or related duties.
  • Or a high school diploma with six years of experience in community work or related duties.
  • Education and/or experience equivalent to the above, with at least one year of qualifying experience.

Responsibilities

  • Coordinate and monitor Relay implementation at assigned hospital EDs and referrals to community services.
  • Provide technical assistance and train ED staff on program policies and procedures.
  • Collaborate with ED staff and community partnerships to integrate Relay.
  • Use lived experience with substance use to support ED patients and obtain consent for follow-up services.
  • Discharge: accompany patients to appointments as required.
  • Conduct overdose prevention trainings with patients and their social networks, including naloxone distribution.
  • Contact and engage patients within 24 hours of hospital discharge.
  • Provide linkage to care and community resources for up to 90 days post-discharge.
