Job summary

The Department of Health and Mental Hygiene’s BADU unit is hiring a Community Coordinator to support the Relay program in NYC hospitals. The role focuses on coordinating care, training ED staff, and connecting patients to ongoing services after overdoses, with emphasis on harm reduction and community partnerships.

You will leverage lived experience and collaborate with diverse teams across boroughs, traveling as needed.

Qualifications

  • Bachelor's degree or higher with two years of related community work experience.
  • High school graduate with six years of related experience.
  • Equivalent combination of education and experience with at least one year in relevant duties.

Responsibilities

  • Coordinate Relay implementation at hospital EDs and referrals to community services.
  • Provide training to ED staff on policy and procedures.
  • Collaborate with ED staff and partners to integrate Relay.
  • Use lived experience to support patients after non-fatal overdoses and obtain consent for follow-up services.
  • Conduct overdose prevention trainings including naloxone distribution.
  • Engage patients within 24 hours after hospital discharge.
  • Assist with linkage to care for up to 90 days post-discharge; accompany to appointments as needed.
  • Offer referrals to harm reduction, MAT and treatment options as requested.
  • Maintain detailed records of contacts and follow-up; report program data.
