Job summary

First Step Communities seeks a Watt Outreach Navigator / Case Manager to serve individuals experiencing homelessness in Sacramento County. You will bridge clients with shelter, housing, behavioral health, and supportive services through outreach, assessment, and referral while maintaining confidentiality and trauma-informed practice.

You will work with partner agencies and community organizations to respond to homelessness concerns, coordinate services, and guide clients toward positive outcomes

Qualifications

• High school diploma or GED required; 60+ college units strongly preferred.
• Bachelor’s degree in social work or related field preferred.
• Experience with unsheltered populations and homeless services is desirable.

Responsibilities

• Maintain continuity of care for Watt Weather Response clients during periods when the shelter is inactive; continue outreach and navigation.
• Manage assigned clients with short-term case management and navigation services.
• Build rapport and empower clients to identify goals and solutions.
• Conduct street outreach in encampments and community settings.
• Connect clients to shelter, housing, behavioral health, substance use, medical, benefits and other services.
• Respond to Sacramento 2-1-1 and other referrals; provide CORE resources information.
• Document client contacts and outcomes in HMIS and program records; complete audits and reports.
