Job summary

Socket.dev is seeking an Outreach Specialist to support ECM and related programs by conducting outreach, enrolling members, and coordinating ongoing care. You will engage with care teams, providers, and community partners to improve access to services and resources in diverse settings, including in-person and telehealth visits.

The role requires Spanish proficiency and strong communication, with travel up to 20% and full-time commitment in California.

Qualifications

• Spanish proficiency required.
• Experience in patient outreach preferred.
• Strong interpersonal and communication skills.
• Ability to work independently while collaborating within a multidisciplinary team.
• Proficiency with EHR systems and MS Office preferred.
• Excellent organizational skills.

Responsibilities

• Conduct outreach to eligible members and facilitate enrollment into ECM.
• Complete enrollments and required documentation per guidelines.
• Maintain ongoing contact to support engagement and continuity of care.
• Coordinate care: scheduling, referrals, services, and follow-up.
• Identify barriers and connect members with resources.
• Support Lead Care Managers with follow-up and documentation.
• Collaborate with providers, organizations, and departments to address needs.
• Perform home and community visits to engage members and assist with care goals.
• Monitor progress and report concerns to care team.
• Maintain accurate documentation of outreach and activities.
• Ensure ECM compliance and privacy/documentation standards.
• Perform other duties as assigned.
