Job summary

Sga-Youth- is seeking a dedicated outreach and case management professional to recruit and maintain a caseload of families participating in Healthy Start programs. You will conduct monthly contacts, provide home-based health education including lactation support, and help families set and update goals on the IFSP.

You will navigate referrals and advocate for culturally responsive services. The role emphasizes working with diverse families, maintaining records, and coordinating with community

Qualifications

• Bachelor’s degree required in Social Work, Early Childhood or related field.
• Experience working with mothers, fathers and families.
• Experience in case management.

Responsibilities

• Recruit and maintain a caseload of up to 50 participants.
• Conduct monthly contacts with each participant.
• Provide home-based health education and lactation support.
• Assist with creating and updating an Individual Family Service Plan quarterly.
• Navigate referrals to clinics and community organizations.
• Discuss infant safety and well-child visits with families.
• Promote culturally and linguistically appropriate services.
