Job summary

LifeLong Medical Care is seeking a Case Manager III to support patients with complex needs in Oakland, including home and clinic visits. You will help develop care plans, coordinate enhanced case management, and connect clients with housing resources and benefits.

The role emphasizes outreach, care coordination, and working within a team to improve health outcomes. This is a temporary position with potential extension and union involvement under SEIU-UHW.

Qualifications

• High School diploma or GED required.
• Experience in community health care or social work preferred.
• Knowledge of housing navigation and public benefits is beneficial.

Responsibilities

• Outreach to patients in home, clinic, and community sites.
• Assess needs and help design patient-centered care plans.
• Lead Enhanced Case Management and coordinate referrals.
• Maintain caseload per program standards and use data systems.
• Provide health education and harm-reduction guidance.
• Assist with housing navigation and access to benefits.
• Collaborate with care team and external partners to reduce barriers.
• Document contacts in required data systems and reports.
