A leading healthcare organization is seeking a remote Care Coordinator to manage follow-up care for members. This role involves assessing treatment compliance, coordinating care plans, and preparing documentation. What to Expect (Job Responsibilities):
- Coordinate follow-up care plan needs for members by scheduling appointments or enrolling them in programs
- Assess member compliance with medical treatment plans via telephone or through on-site visits
- Identify barriers to plan compliance and coordinate resolutions
- Recommend treatment plan modifications and determine the need for additional services in conjunction with case management and providers
- Prepare reports to document case and compliance updates
What is Required (Qualifications):
- High School diploma or equivalent
- Minimum of 1 year related experience or a combination of education and experience that provides an equivalent background
- Strong oral, written, and interpersonal communication skills
- Problem-solving, facilitation, and analytical skills
- Ability to work in a virtual environment with minimal supervision
How to Stand Out (Preferred Qualifications):
- Certification as a nurse assistant or medical assistant and/or a BS/BA degree in a related field
- Bilingual skills are preferred
- Experience in healthcare or care coordination roles
- Familiarity with URAC accredited areas and associated competencies
- Previous experience with member outreach or health coaching
