Qualifications

• Must hold or be eligible for CA RN license.
• BSN required; master's preferred.
• At least 1 year transplant coordinator experience.
• 2 years organ procurement and 2 years critical care experience if applicable.
• Excellent communication skills in English.

Responsibilities

• Coordinate outpatient transplant patient care, participate in clinic visits and analyze tests.
• Monitor patient and family adherence to medical regimens.
• Maintain UNOS waiting list and donor/recipient communications.
• Assist in education programs for staff and patients.
• Ensure compliance with CMS, OPTN/UNOS policies and regulatory requirements.
• Prepare documentation for transplant candidacy, listing, and follow-up.
