Job summary

Axess Family Services seeks an Outpatient Therapist to deliver high-quality behavioral health services in an outpatient setting. You will assess, diagnose, and treat individuals, families, and groups using a person-centered, trauma-informed approach.

The role involves treatment planning, documentation, and collaboration with an interdisciplinary team to ensure continuity of care in Ohio. Strong knowledge of evidence-based modalities and HIPAA compliance is essential.

Qualifications

• Master’s degree in counseling or social work (required).
• Knowledge of evidence-based treatment models and recovery-oriented care.
• Strong clinical documentation and organizational skills.
• Ability to work independently and as part of a multidisciplinary team.
• Ability to meet agency driving insurability requirements.

Responsibilities

• Conduct comprehensive behavioral health assessments, including diagnostic evaluations and psychosocial assessments.
• Develop, implement, and regularly update individualized treatment plans with clients and the interdisciplinary team.
• Provide individual, family, and group psychotherapy aligned with licensure.
• Utilize evidence-based modalities (e.g., CBT, DBT, MI, trauma-informed care).
• Maintain accurate clinical documentation in the EHR per OMHAS and Medicaid standards.
• Participate in interdisciplinary meetings, case consultations, and care coordination.
• Collaborate with providers and community partners to support continuity of care.
• Provide crisis intervention, safety planning, and appropriate higher-level referrals when needed.
• Maintain ethical standards and client confidentiality per HIPAA and 42 CFR Part 2.
