Chartiers Center, Inc. in Washington County Clinic, Washington, PA 15301 seeks a Master’s level therapist or LPC to provide outpatient child therapy.

The role includes evaluations, treatment planning, and coordination with treatment teams to support adults with mental health challenges and complex diagnoses. The position requires a Master’s degree or LPC and two years’ experience preferred, with benefits including medical, dental, vision, disability, life insurance, retirement, and tuition

• Specialties: SPMI, Dual Diagnosis MH/D&A, Dual Diagnosis MH/IDD.

Responsibilities

• Complete required evaluation and assessment for treatment of consumers.
• Complete all required documentation including assessments, service plans and progress notes.
• Plan and coordinate treatment through treatment team meetings and supervision.
• Develop and implement individualized treatment plans for consumers.
• Maintain ongoing contact with Service Coordinators and other program staff to ensure continuity of care.

ChartiersCenter is a private, non-profit corporation funded by state, county, third partyinsurancesand private funds. Services include intake, referral, outpatient therapy, psychiatric assessment, service coordination, substance abuse treatment, training and social rehabilitative services, crisis intervention, partial hospitalization,community outreach, day, and recreational services foradultswith intellectual disabilities, homeless outreach, and housing program.

Asan Outpatient Child Therapistwith Chartiers Centerat ourWashington County Clinic, youwould becommitted to providingcompassionatesupportsand therapy topeople facing mental health challenges.Job duties as atherapist include:

• Complete required evaluation and assessment for treatment of consumers
• Complete all required documentation to include, but not limited to managed care information, initial consumer assessments, service plans, service plan reviews, progress notes, correspondence, transfer summariesand terminations summaries.
• Plan and coordinate treatment for consumers through participation in treatment team meetings, interdisciplinary team meetings, psychiatric staffing, and individual supervision.
• Develop and implement individualized treatment plans for an assignedconsumers utilizing goal-setting techniques.
• Maintain ongoing contact with Service Coordinators and other Agency program staff to ensure consumer accessibility of community/supportive services and continuity ofcare.
