Chartiers Center, Inc. is recruiting for a Mobile Psychiatric Rehabilitation Worker to support clients in home and community settings.

You will help individuals develop independent living, social, and job-search skills while coordinating with supports and resources. The role emphasizes frequent in-person visits, detailed documentation, and a pathway to advancement via CPRP with the potential transition to a Psychiatric Rehabilitation Specialist.

Qualifications

• An associate degree and 1 year work experience in mental health direct service.
• A CPS certificate and 1 additional year work experience in mental health direct service.
• A High School Diploma or GED and 2 years work experience in human services which must include 1 year of mental health direct service.
• A Bachelor's Degree and preferably 1 year work experience in mental health direct service.
• Valid driver's license required.

Responsibilities

• Meet regularly with a caseload of clients in their home or community to teach them needed skills such as independent living skills, social skills, and job search skills, and help connect them with supports such as needed living resources and social opportunities.
• Maintain a weekly schedule where the majority of your work time is spent in face-to-face client meetings.
• Thoroughly document client meetings to reflect work completed and interventions delivered, as well as client’s response to service and any client concerns.

Job Duties include
