Job summary

Easterseals PORT Health in Elizabeth City, NC is seeking an Outpatient Therapist to support clients with mental health and substance use needs. You will conduct assessments, develop goals, maintain records, and coordinate care with families and agencies.

Qualifications

• Master’s degree in psychology, social work, or related field with current licensure.
• LCSW-A, LCMHC-A, or LCAS-A required; dual licensure preferred.
• Experience in substance use services preferred.
• Proficient with EHR systems.
• Valid driver’s license, insurance, and reliable transportation.

Responsibilities

• Conduct comprehensive screenings, diagnostic assessments, and evaluations.
• Develop person-centered treatment goals with clients and families.
• Maintain medical, social, and clinical information accurately.
• Review pre-screening information and communicate with referral sources.
• Collaborate with staff and partners to determine level of care.
• Liaise with local agencies to support access and continuity of care.
• Facilitate psychoeducation, groups, activities, and family sessions.
• Provide therapeutic interactions emphasizing support and best practices.

Pay: Salary | $65,000 and up / provisionally licensed & up to $78,000 for fully licensed candidates

What You’ll Do

• Conduct comprehensive screenings, diagnostic assessments, and evaluations for individuals presenting with mental health, substance use, or psychiatric symptoms.
• Develop person-centered treatment goals and plans in collaboration with clients and families.
• Gather, documents and maintain all required medical, social, and clinical information in a timely and clinically accurate manner.
• Review pre-screening information from referral sources and communicate directly with referral agencies as needed.
• Collaborate with co-workers, clinical supervisors, physicians, and community partners to determine the appropriate level of care within the service continuum.
• Serve as a liaison with local agencies, community partners, and referral networks to support client access and continuity of care.
• Facilitate psychoeducation, psychotherapy groups, structured activities, and family education sessions.
• Provide therapeutic interactions that emphasize support, guidance, and clinical best practices.

What You’ll Bring

• Education: A Master’s degree and mental health licensure are required.
  • LCSW (A), LCMHC (A), LCAS (A);
  • Dual licensure preferred.
• Experience in Substance Use services preferred.
• Proficiency with EHR systems or technically savvy.
• Valid driver’s license, insurance and reliable transportation for work-related travel.
