Job summary

Easterseals PORT Health is hiring a full-time clinical professional in Whiteville, NC to conduct screenings, assessments, and develop person-centered treatment plans for individuals with mental health or substance use needs.

You will document thoroughly, coordinate care with families and partners, and contribute to a compassionate, growth‑oriented team focused on improving lives.���

Qualifications

• Master’s degree in a mental health field is required.
• Current licensure (LCSW, LCMHC, or LMFT); dual LCAS preferred.
• Proficiency with electronic health record systems.
• Valid driver’s license and reliable transportation for work-related travel.

Responsibilities

• Conduct comprehensive screenings, diagnostic assessments, and evaluations for mental health, substance use, or psychiatric symptoms.
• Develop person-centered treatment goals and plans with clients and families.
• Document and maintain all required medical, social, and clinical information timely and accurately.
• Review pre-screening information from referrals and communicate with referral agencies as needed.
• Collaborate with clinicians, supervisors, physicians, and partners to determine appropriate level of care.
• Serve as liaison with local agencies and networks to support access and continuity of care.
• Facilitate psychoeducation, psychotherapy groups, and family education sessions.

Pay: $55,000 – $57,000 Salary | based on experience

What You’ll Do

• Conducts comprehensive screenings, diagnostic assessments, and evaluations for individuals presenting with mental health, substance use, or psychiatric symptoms.
• Develops person-centered treatment goals and plans in collaboration with clients and families.
• Gathers, documents, and maintains all required medical, social, and clinical information in a timely and clinically accurate manner.
• Reviews pre-screening information from referral sources and communicates directly with referral agencies as needed.
• Collaborates with co-workers, clinical supervisors, physicians, and community partners to determine the appropriate level of care within the service continuum.
• Serves as a liaison with local agencies, community partners, and referral networks to support client access and continuity of care.
• Facilitates psychoeducation, psychotherapy groups, structured activities, and family education sessions.

What You’ll Bring

• Education: A Master’s degree and mental health licensure are required.
  • LCSW (A), LCMHC (A), LMFT (A);
  • Dual licensure preferred LCAS (A)
• Proficiency with EHR systems or technically savvy.
• Valid driver’s license, insurance and reliable transportation for work-related travel
