As a leader and integrator of care, the RN plans, coordinates, and evaluates patient care needs, collaborating with the patient and family to ensure high-quality, evidence-based outcomes in an outpatient setting.

Qualifications

• RN licensure and current unrestricted practice in Minnesota.

Responsibilities

• Assessment: gather comprehensive data from patient, family, and environment.
• Diagnosis: interpret data to identify nursing needs and plan care.
• Outcomes Identification: set measurable goals with patient and family.
• Planning: develop a care plan with patient and family for continuity of care.
• Implementation: execute the plan with safe, timely interventions.
• Evaluation: reassess progress and modify plan as needed.
• Quality of Practice: contribute to ongoing nursing quality improvement.
• Education: pursue ongoing knowledge and competencies in nursing.
• Professional Practice Evaluation: self-evaluate practice against standards.
