Job summary
Allina Health is seeking a registered nurse to join our remote triage program. This position supports centralized nursing services, including triage, anticoagulation, and refill, in an outpatient setting. The role is 0.8 FTE (64 hours per two-week pay period) with a day shift and every 4th weekend rotation, fully remote with occasional onsite orientation.

Qualifications
- Associate's or Vocational degree in nursing.
- Minimum 3 years RN experience.
- Licensed Registered Nurse in MN and WI required by orientation
Responsibilities
- Assessment: collect and synthesize comprehensive patient data.
- Diagnosis: analyze data to identify nursing diagnoses and plan care.
- Outcomes Identification: establish realistic, measurable patient outcomes with family.
- Planning: develop and coordinate an individualized plan of care.
- Implementation: execute the plan with evidence-based interventions.
- Evaluation: assess progress and revise diagnoses/outcomes as needed.
