Job summary

Trinity Health Alliance of Michigan is seeking an RN Care Manager to oversee high-risk members' care. You will assess, plan, intervene, monitor, and document across the continuum, coordinating with physicians and care teams to prevent inpatient readmissions.

The role requires a BSN, Michigan RN license, CPR, a valid Michigan driver's license, and at least three years in acute care or case management. A fast-paced environment and flexible scheduling are essential.

Qualifications

• Graduation from an accredited BSN program.
• Current Michigan RN license, CPR certification, valid Michigan driver's license.
• Three years acute care or case management experience.
• Preferred: two years utilization or HMO experience.
• Ability to work in a fast-paced environment with multiple tasks.
• May involve exposure to infectious disease and hazardous materials.
• Ability to work flexible schedules.

Responsibilities

• Keeps member/family informed and requests further assistance as needed.
• Follows through with requests and communicates critical information timely.
• Acts as liaison between administration, members, physicians and providers.
• Involves member and family in forming the plan of care.
• Performs Health Risk Assessments and determines acuity scores.
• Develops and implements an outcome-based plan of care.
• Documents assessments, care plans, and pertinent information in medical records.
• Maintains workflow to meet NCQA standards and manages caseload.
