Trinity Health Alliance of Michigan in Southeast Michigan is seeking an RN Care Manager to join our in-person team. The role focuses on assessment, care planning, intervention, and coordination to support high-risk members and reduce hospital readmissions.
The ideal candidate holds a BSN, Michigan RN license with CPR, and at least three years in acute care or case management; familiarity with utilization or HMO is a plus. Onsite work with no weekends or on-call required.

Position Summary: Performs assessment, treatment planning, intervention, monitoring, evaluation and documentation on identified High Risk members. Assesses and develops a care plan in collaboration with the admitting, attending and consulting physician, the member and other health care practitioners. Effectively manages members on an outpatient basis to ensure provision of an appropriate level of care, to minimize inpatient re‑admissions, and to ensure that members’ medical, environmental and psychosocial needs are met across the continuum of care.
