Job summary

Qualifications

Graduation from an accredited BSN program.
Current Michigan RN license, CPR certification, and valid Michigan driver's license.
Three years acute care or case management experience.
Preferred: two years utilization or HMO experience.
Fast-paced environment requires multitasking and professional conduct.

Responsibilities

Keeps member/family informed and requests further assistance when needed.
Acts as liaison between administration, members, physicians and other providers.
Performs Health Risk Assessment (HRA) and determines an acuity score for follow-up.
Develops an outcome-based plan of care and documents it in the member’s medical record.
Maintains NCQA-aligned workflow and manages patient caseload to meet goals.
