Purpose And Scope:

The professional registered nurse Outpatient RN CAP 1 is an entry level designation into the Clinical Advancement Program (CAP). This position is accountable for providing and coordinating clinically competent care, including assessment, planning, intervention, and evaluation for an assigned group of patients. It may include delegating appropriate tasks to direct patient care staff such as RNs, LVN/LPNs, and Patient Care Technicians. As a member of the End Stage Kidney Disease (ESKD) healthcare team, this role involves decision-making, teaching, leadership, and quality improvement activities to enhance patient outcomes and facility operations.

Principal Duties and Responsibilities:
  • Perform all essential functions under supervision and guidance from the Clinical Educator, Preceptor, or collaborating RNs.
  • Collect and analyze patient data before, during, and after hemodialysis, document findings, and adjust treatment plans accordingly.
  • Assess and document patient/family learning needs, providing ongoing education.
  • Collaborate with the care team to deliver safe and effective hemodialysis therapy.
  • Administer medications as prescribed, documenting medical justification and effectiveness.
  • Assist with emergency response measures as needed.
  • Participate in staff training, orientation, and serve as a resource for the healthcare team.
  • Ensure proper laboratory procedures and reschedule missed collections.
  • Update nursing assessments and care plans through collaboration with the interdisciplinary team.
  • Inform patients about transplant and treatment options.
  • Complete CAP requirements for advancement.
  • Perform other duties as assigned by the Supervisor.
Physical Demands and Working Conditions:
  • Involves heavy lifting, patient movement, and assisting with ambulation.
  • Requires prolonged standing and bending.
  • May involve moving equipment weighing up to 200 lbs. and lifting solutions up to 30 lbs.
  • Work environment is a healthcare setting with controlled temperature and moderate noise.
  • Potential exposure to infectious materials.
  • Includes desk work, computer tasks, and interaction with staff and patients.
  • May require travel to training sites or other facilities.
Supervision:
  • Oversight of Patient Care Technicians, LPNs, LVNs, or RNs as a Team Leader or Nurse in Charge, after meeting certain experience and training requirements.
Education and Licensure:
  • Graduate of an accredited School of Nursing.
  • Current state licensure.
  • CPR/BLS certification.
  • Meet practice requirements in the employment state.
Experience and Skills:
  • Entry level for RNs with less than 2 years of nephrology nursing experience.
  • Chronic/acute hemodialysis experience preferred.
