Job Title: Outpatient Registered Nurse - RN

Purpose and Scope

The professional registered nurse Outpatient RN CAP 1 is an entry-level position within the Clinical Advancement Program (CAP). This role involves providing and coordinating clinically competent care, including assessment, planning, intervention, and evaluation for an assigned group of patients. Responsibilities may include delegating tasks to direct care staff such as RNs, LVNs/LPNs, and Patient Care Technicians. As part of the End Stage Kidney Disease (ESKD) healthcare team, the RN participates in decision-making, teaching, leadership, and quality improvement activities aimed at enhancing patient outcomes and facility operations.

Principal Duties and Responsibilities

  • Perform essential functions under supervision and guidance from the Clinical Educator, Preceptor, or collaborating RNs.
  • Collect and analyze patient data systematically before, during, and after hemodialysis treatments, document findings, and adjust treatment plans as needed.
  • Assess and document patient and family learning needs to provide ongoing education.
  • Ensure safe and effective delivery of hemodialysis therapy in collaboration with care staff.
  • Administer medications as prescribed, documenting medical justification and effectiveness.
  • Assist with emergency response measures as needed.
  • Serve as a resource to the healthcare team and participate in staff training and orientation.
  • Ensure proper laboratory procedures and reschedule missed collections.
  • Update nursing assessments and care plans collaboratively with the interdisciplinary team.
  • Educate patients about transplant and treatment options.
  • Complete CAP requirements for advancement.
  • Perform other duties as assigned by the Supervisor.

Physical Demands and Working Conditions

  • Involves heavy lifting, moving patients, and assisting with ambulation, with equipment and coworker assistance as needed.
  • Requires prolonged standing and bending.
  • May involve moving equipment up to 200 lbs. and lifting solutions up to 30 lbs. to heights of 5 feet.
  • Work environment typical of healthcare facilities, with exposure to infectious materials.
  • Includes desk work, computer interaction, and collaboration with patients and staff.
  • May require travel and work in different locations with similar physical demands.

Supervision

  • Oversight of Patient Care Technicians, LPNs, LVNs, or RNs as a Team Leader or Nurse in Charge, following specific experience and training prerequisites.

Education and Licensure

  • Graduate of an accredited School of Nursing.
  • Current state licensure.
  • CPR/BLS certification.
  • Meet state practice requirements.

Experience and Skills

  • Entry-level for RNs with less than 1 year of recent Nephrology Nursing experience.
  • Minimum of 9 months RN experience preferred.
  • Chronic/acute hemodialysis experience preferred.
