Purpose and Scope

The professional registered nurse Outpatient RN CAP 2 may be an entry-level position into the Clinical Advancement Program (CAP) for new employees who meet the RN CAP 2 criteria or who have advanced from RN CAP 1. This role involves providing and coordinating clinically competent care, including assessment, planning, intervention, and evaluation for assigned patients. It may include delegating tasks to staff such as RNs, LVN/LPNs, and Patient Care Technicians. As part of the End Stage Kidney Disease (ESKD) team, the RN participates in decision-making, teaching, leadership, and quality improvement activities to enhance patient outcomes and facility operations.

Principal Duties and Responsibilities
  • Perform functions under supervision and guidance from educators, preceptors, or other RNs.
  • Collect and analyze patient data before, during, and after hemodialysis; document findings and adjust treatment plans accordingly.
  • Assess and educate patients and families about care needs and treatment options.
  • Ensure safe and effective delivery of hemodialysis therapy in collaboration with care staff.
  • Administer medications as prescribed, document, and evaluate effectiveness.
  • Assist with emergency responses as needed.
  • Serve as a resource and mentor for team members, modeling appropriate behavior and work habits.
  • Ensure proper laboratory procedures and follow up on missed collections.
  • Update nursing assessments and care plans collaboratively with the interdisciplinary team.
  • Participate in educational and quality improvement initiatives.
  • May serve as a preceptor for new staff.
  • Complete CAP requirements to maintain or advance certification.
  • Perform other duties as assigned by the supervisor.
Physical Demands and Working Conditions

This role involves physical activities such as heavy lifting, moving patients, prolonged standing, and assisting with ambulation. Equipment aids and team assistance are available. The environment is typical of a healthcare setting, with potential exposure to infectious materials. The work includes desk and computer tasks, with possible travel to training or other facilities.

Supervision
  • Oversee Patient Care Technicians, LVN/LPNs, and RNs as a Team Leader or Nurse in Charge, upon meeting specific training and experience criteria.
Education and Licensure
  • Graduate of an accredited School of Nursing.
  • Current state licensure.
  • CPR/BLS certification.
Experience and Skills
  • Minimum 2 years of Nephrology Nursing experience; entry-level for RNs with this experience.
