Job Description: Outpatient Registered Nurse - RN at RemoteWorker US

Purpose and Scope

The professional registered nurse Outpatient RN CAP 2 may be an entry-level designation into the Clinical Advancement Program (CAP) for new employees who meet the RN CAP 2 criteria or attained through advancement from RN CAP 1. This position is accountable for providing and coordinating clinically competent care, including assessment, planning, intervention, and evaluation for an assigned group of patients. It may include delegation of appropriate tasks to direct patient care staff, such as RNs, LVN/LPNs, and Patient Care Technicians. As part of the End Stage Kidney Disease (ESKD) healthcare team, this role involves decision-making, teaching, leadership, and quality improvement activities to enhance patient outcomes and facility operations.

Principal Duties and Responsibilities

  • Perform essential functions under supervision and guidance from the Clinical Educator, Preceptor, or other Registered Nurses.
  • Collect and analyze patient data before, during, and after hemodialysis treatment; document findings; adjust treatment plans as needed; notify appropriate team members.
  • Assess and document patient/family learning needs; provide education on treatment options and care.
  • Coordinate and deliver safe hemodialysis therapy in collaboration with care staff.
  • Administer medications as prescribed and document their effectiveness.
  • Initiate or assist with emergency response measures.
  • Serve as a resource, mentor, and role model for staff and new employees.
  • Ensure proper laboratory procedures are followed and reschedule missed collections.
  • Update nursing assessments and care plans through collaboration with the interdisciplinary team.
  • Participate in educational and quality improvement initiatives.
  • May serve as a Preceptor to new staff.
  • Complete CAP requirements for career advancement.
  • Perform other duties as assigned by the supervisor.

Physical Demands and Working Conditions

  • Involves heavy lifting, moving patients, and assisting with ambulation, with equipment aids or assistance.
  • Requires prolonged standing and bending.
  • May involve moving equipment up to 200 lbs. and lifting solutions up to 30 lbs.
  • Work environment typical of healthcare facilities, with exposure to infectious materials.
  • Includes desk work, computer use, and interaction with patients and staff.
  • Travel may be required to training sites or other facilities.

Supervision

  • Oversee Patient Care Technicians, LVN/LPNs, or RNs as a Team Leader or Nurse in Charge, after meeting training and experience requirements.

Education and Licensure

  • Graduate of an accredited School of Nursing.
  • Current state licensure.
  • CPR/BLS certification.
  • Meet practice requirements of the state of employment.

  • Entry-level RN with at least 2 years of Nephrology Nursing experience preferred.
