Purpose And Scope: The professional registered nurse Outpatient RN CAP 1 is an entry level designation into the Clinical Advancement Program (CAP). This position is accountable for providing and coordinating clinically competent care, including assessment, planning, intervention, and evaluation for an assigned group of patients. It may involve delegating appropriate tasks to direct patient care staff such as RNs, LVN/LPNs, and Patient Care Technicians. As part of the End Stage Kidney Disease (ESKD) healthcare team, this role participates in decision-making, teaching, leadership, and quality improvement activities to enhance patient care outcomes and facility operations.

Principal Duties and Responsibilities:
  1. Perform all essential functions under the supervision of the Supervisor and with guidance from the Clinical Educator, Preceptor, or in collaboration with other Registered Nurses.
  2. Collect and analyze patient data systematically before, during, and after hemodialysis treatments, document findings, adjust treatment plans as needed, and notify relevant team members.
  3. Assess and document patient/family learning needs, providing ongoing education about treatments and care.
  4. Collaborate with staff to ensure safe and effective delivery of hemodialysis therapy.
  5. Administer medications as prescribed, documenting medical justification and effectiveness.
  6. Initiate or assist with emergency response measures.
  7. Participate in staff training, orientation, and serve as a resource for the healthcare team.
  8. Ensure proper laboratory procedures and reschedule missed collections.
  9. Update nursing assessments and care plans in collaboration with the interdisciplinary team.
  10. Provide information to patients regarding transplant and treatment options.
  11. Complete CAP requirements for professional development.
  12. Perform other duties as assigned by the Supervisor.
Physical Demands and Working Conditions:
  • Involves heavy lifting, patient mobility assistance, and prolonged standing.
  • May require moving equipment up to 200 lbs. and lifting solutions up to 30 lbs.
  • Work environment typical of healthcare facilities, with potential exposure to infectious materials.
  • Includes desk work, computer use, and interaction with staff and patients.
  • May require travel to training or other facilities.
Supervision:
  • Oversee Patient Care Technicians, LPNs, LVNs, or RNs as a Team Leader or Nurse in Charge, after meeting specific experience and training requirements.
Education and Licensure:
  • Graduate of an accredited School of Nursing.
  • Current state nursing license.
  • CPR/BLS certification.
  • Meet state practice requirements.
Experience and Skills:
  • Entry level for RNs with less than 2 years of nephrology nursing experience.
  • Preferred: experience in chronic/acute hemodialysis.

Transitional Care Unit - RN:

Support patients in moving to Home Hemodialysis treatments.
