Purpose And Scope:

The professional registered nurse Outpatient RN CAP 1 is an entry level designation into the Clinical Advancement Program (CAP). This position is accountable for providing and coordinating clinically competent care, including assessment, planning, intervention, and evaluation for an assigned group of patients. It may include delegating appropriate tasks to direct patient care staff such as RNs, LVN/LPNs, and Patient Care Technicians. As a member of the End Stage Kidney Disease (ESKD) healthcare team, this role involves decision-making, teaching, leadership, and quality improvement activities to enhance patient care outcomes and facility operations.

Principal Duties And Responsibilities:
  • Perform all essential functions under supervision and guidance from the Clinical Educator, Preceptor, or collaborating RNs.
  • Collect and analyze patient data pre-, during-, and post-hemodialysis, document findings, and adjust treatment plans accordingly, notifying the appropriate team members as needed.
  • Assess and document patient/family learning needs, providing ongoing education.
  • Deliver safe and effective hemodialysis therapy in collaboration with care staff.
  • Administer prescribed medications, document medical justifications, and monitor effectiveness.
  • Initiate or assist with emergency response measures.
  • Serve as a resource, participate in staff training, and orient new staff.
  • Ensure proper laboratory procedures and reschedule missed collections.
  • Update nursing assessments and care plans collaboratively with the interdisciplinary team.
  • Inform patients about transplant and treatment options.
  • Complete CAP requirements for advancement.
  • Perform other duties as assigned.
Physical Demands And Working Conditions:

Includes heavy lifting, patient movement, prolonged standing, equipment handling, and exposure to infectious materials, typical of healthcare settings. Reasonable accommodations are available for individuals with disabilities.

Supervision:

Oversee Patient Care Technicians, LPNs, LVNs, or RNs as a Team Leader or Nurse in Charge, after meeting specific experience and training requirements.

Education And Licensure:
  • Graduate of an accredited School of Nursing.
  • Current state licensure.
  • CPR/BLS Certification.
  • Meet state practice requirements.
Experience And Skills:
  • Entry level RN with less than 2 years of Nephrology Nursing experience.
  • Preferred: experience in chronic/acute hemodialysis.
