United Future is seeking an Outpatient Physical Therapist to join a supportive rehabilitation team in Cayuga, NY. The role focuses on hands-on care in a busy outpatient clinic, with a broad mix of patient needs and a commitment to quality life improvements.

Qualifications

• Experience in outpatient physical therapy settings.
• Must hold or be eligible to obtain a valid Physical Therapist license in the state at the time of hire.
• A Doctor of Physical Therapy (DPT) or equivalent degree from an accredited program is required.
• Basic Life Support (BLS) certification required within 90 days of hire.
• Strong communication, collaboration, organizational and time-management skills.
• Ability to deliver high-quality, compassionate care for a wide range of patients.
• Must be able to lift up to 30 pounds independently and up to 150 pounds with assistance.

Responsibilities

• Conduct initial evaluations and develop individualized treatment plans.
• Administer therapies including exercises, manual therapy, modalities, and patient education.
• Educate patients on exercise programs, posture correction, and injury prevention.
• Collaborate with the multidisciplinary care team for comprehensive care.
• Monitor patient progress and adjust treatment plans as needed.
• Document treatment plans, progress notes, and discharge summaries in compliance with policies.
• Provide patient education to empower rehabilitation and injury prevention.
• Maintain a clean, safe, and organized treatment area.
