Job summary

Millennium Physician Group in Cape Coral, FL, seeks a dedicated physician to provide comprehensive outpatient primary care within a value-based care model. You’ll develop long-term relationships with adults, focusing on preventive care and chronic disease management.

You will collaborate with an integrated care team and utilize AthenaOne EMR to support evidence-based care, while participating in quality initiatives and pursuing patient-centered outcomes.

Qualifications

• MD or DO with Florida license or ability to obtain.
• Board Certified or Board Eligible in Family Medicine or Internal Medicine.
• Active Florida medical license (or ability to obtain); DEA license.
• Passion for value-based, patient-centered care; English and Spanish a plus.

Responsibilities

• Provide comprehensive outpatient primary care for adult patients.
• Develop long-term relationships focused on preventive care and chronic disease management.
• Collaborate with interdisciplinary care teams to improve quality outcomes.
