BAYADA Home Health Care is currently seeking an experienced Physical Therapist (PT) for a full-time opportunity providing outpatient home health visits for our senior living office located in Harrisburg, PA. This office services adult and geriatric clients across territories in York County.

Requirements include one year of clinical experience as a licensed Physical Therapist. As a home care Physical Therapist, you will be part of a multidisciplinary team delivering rehabilitative care and skilled nursing, helping clients remain at home.

Responsibilities:

1. Make outpatient visits to clients within designated territories.
2. Perform diagnostic tests and measurements (e.g., mobility, strength, balance, safety).
3. Develop and implement individualized care plans, including manual therapy, gait training, and balance interventions.
4. Assess and revise care plans, collaborating with nursing staff.
5. Educate clients and families on rehabilitative activities to promote independence.
6. Document care activities accurately using electronic tablets.

Qualifications:

• Current PA Physical Therapist (PT) license.
• At least one year of recent clinical experience as a PT.
• Graduation from an APTA-approved program.
• Ability to work independently and manage time effectively.
• Strong interpersonal and organizational skills.
• Proficiency with computers; EMR experience preferred.
