Qualifications

• Graduate of an accredited physical therapy program; Doctorate degree preferred.
• Current license to practice Physical Therapy in the state of Ohio; BLS certification required.
• One year of clinical experience preferred.
• Effective communication skills.
• Ability to travel to other sites as necessary.

Responsibilities

• Performs PT evaluation and establishes treatment plan for referred patients.
• Documents PT evaluation results, treatments, progress, and discharge records.
• Maintains proper billing for services provided.
• Delegates to PTA and co-signs their documentation.
• Communicates with other health care providers regarding treatment and discharge planning.
