Job summary

Mount Carmel Health System in Ohio is seeking a Physical Therapist to evaluate patients and implement treatment plans, collaborating with other caregivers to ensure quality care and maintain accurate patient records. The role requires graduation from an accredited PT program (Doctorate preferred), Ohio PT licensure, and current BLS certification; travel to other Mount Carmel sites may be required.

You will document results, participate in discharge planning, and contribute to quality improvement

Qualifications

• Graduate of an accredited physical therapy program; Doctorate preferred.
• Current license to practice Physical Therapy in Ohio; BLS required.
• One year of clinical experience preferred.

Responsibilities

• Performs PT evaluation and establishes treatment plan for patients.
• Documents evaluation results, interventions, progress and discharge records.
• Maintains proper billing for services; delegates to PTA and reviews their docs.
• Communicates with health care providers regarding treatment, continuity of care, and discharge planning.
