Job summary

Mount Carmel Health System invites qualified Physical Therapists to join our team in Central Ohio. The role focuses on evaluating patients, developing and executing treatment plans, and coordinating with the care team to ensure seamless discharge planning.

The position emphasizes accurate documentation, proper billing, and effective communication with providers. Ohio licensure and BLS are required, with potential travel to other Mount Carmel sites as needed.

Qualifications

• Graduate of an accredited physical therapy program; Doctorate preferred.
• Current license to practice Physical Therapy in Ohio.
• Current BLS for Health Care Provider certification.
• One year of clinical experience preferred.
• Strong communication and judgment in patient evaluation and treatment.

Responsibilities

• Perform PT evaluation and develop/implement treatment plans.
• Document PT evaluations, interventions, progress, and discharge records.
• Maintain proper billing for services provided.
• Delegate tasks to PTA; review and co-sign documentation.
• Communicate with health care providers regarding patient treatment and discharge planning.

Description:

Position Purpose

The Physical Therapist provides evaluation and treatment of patients referred for physical therapy services. The Physical Therapist communicates and collaborates with other caregivers to provide quality care and maintains accurate patient records.

What You Will Do

• Performs physical therapy evaluation, establishes and implements treatment plan for patients referred by a provider and assigned by the manager, clinical coordinator or designee.
• Documents accurately PT evaluation results, treatment interventions, patient progress toward goals, and discharge records.
• Maintains proper billing for the services provided.
• Delegates appropriate treatment procedures to the Physical Therapist Assistant/PTA; reviews/co-signs their documentation.
• Communicates with appropriate health care providers on matters concerning patient treatment, continuity of care, and discharge planning.

Minimum Qualifications

• Education: Graduate of an accredited physical therapy program; Doctorate degree preferred.
• Licensure / Certification: Current license to practice Physical Therapy in the state of Ohio; Current Basic Life Support (BLS) for Health Care Provider certification required.
• Experience: One year of clinical experience in respective area preferred.
• Effective Communication Skills
• Ability to make decisions and utilize good judgment concerning patient evaluation and treatment.
• Ability to function in a professional capacity with minimal supervision.
• Physically able to perform strenuous activities, such as lifting and transferring patients.
• Ability to travel to other sites as necessary to meet system needs.
