Qualifications

• Graduate of an accredited Physical Therapy program.
• Current or eligible Illinois Physical Therapist license.

Responsibilities

• Provide outpatient PT services with 60-minute evaluations and 30-minute follow-ups in Lockport, IL.
• Perform evaluations, documentation, and treatment planning with patient-centered care.
