Job summary

Duly Health and Care in New Lenox, IL is seeking a Physical Therapist I to join our outpatient rehabilitation team. You will work with PTs, OTs, PTAs, physicians, and APPs to deliver patient-centered care in a collaborative setting.

The role features a Mon–Fri schedule with two evenings per week until 7 PM, 60-minute evaluations, built-in documentation time, mentorship, and access to continuing education. Competitive base pay with a sign-on bonus and comprehensive benefits available.

Qualifications

• Degree in Physical Therapy from an accredited curriculum.
• Certification in Physical Therapy and valid state license.

Responsibilities

• Collaborate with PTs, OTs, PTAs, physicians, and APPs in a team-based setting.
• Provide care with a schedule that prioritizes quality over volume, Mon–Fri with two evenings until 7 PM.
• Perform 60-minute evaluations with no double booking and 30-minute follow-ups.
• Include built-in documentation time to support work-life balance.
• Participate in mentorship, continuing education, and specialty interest groups.
• Contribute to leadership, education, and career growth pathways.
