Elliot Health System is seeking a Physical Therapist for outpatient rehabilitation in Manchester, NH. The role emphasizes delivering high-quality therapy, evaluating patients, and developing individualized plans of care within an outpatient setting.

Formación

• Graduate of an accredited Physical Therapy Program with corresponding entry-level degree.
• Licensed as a Physical Therapist by the state of New Hampshire.
• CPR certification required or able to obtain within 90 days.
• Recent graduates are eligible.

Responsabilidades

• Evaluates and plans individualized treatment programs for outpatient rehab.
• Coordinates with health care team to develop short- and long-term goals.
• Updates treatment plans and communicates with referrer and other clinicians.
• Provides discharge planning and home exercise instructions.
• Supervises therapy assistants and rehab staff as needed.
• Participates in ongoing professional development and education programs.
